Your MRI Is “Clean”… So Why Are You Still in Pain?

You did what you were supposed to do.
You went to the doctor. You got the MRI. You waited for the results, hoping to finally have an explanation for the pain that’s been keeping you from training, working, sleeping, or simply moving the way you want to.
Then you hear:
“Everything looks normal.”
That should feel like good news. And in many ways, it is.
But if your MRI came back normal and you’re still in pain, it can leave you with an even bigger question:
If nothing showed up on my MRI, why do I still hurt?
The answer may be that your MRI and your symptoms are giving us two different pieces of the picture.
Can You Still Have Pain With a Normal MRI?
Yes. An MRI can provide valuable information about the structures inside your body, but imaging is only one part of understanding why someone may be experiencing pain.
MRIs can help healthcare providers identify structural findings such as disc changes, ligament injuries, inflammation, and other abnormalities.
But an MRI is still a snapshot.
It shows what your body looks like while you’re lying relatively still. It doesn’t show us everything that happens when you squat, rotate, run, reach overhead, pick something up, or perform the activity that actually brings on your symptoms.
That distinction matters.
Why Pain May Not Show Up on an MRI
It’s easy to assume that if something hurts, there must be damaged tissue somewhere causing the pain.
But pain is more complex than that.
Symptoms can be influenced by many factors, including how your joints move, how different areas of your body work together, how you tolerate load, and how your nervous system responds to movement and certain positions.
This means someone can experience very real pain even when their imaging doesn’t reveal an obvious structural explanation for those symptoms.
The opposite can happen, too. Imaging can identify structural changes in people who aren't experiencing pain.
An MRI can be an important piece of the puzzle. It just isn't always the entire puzzle.
Sometimes the Answer Shows Up When You Move
This is where a movement-based assessment can provide additional information.
Let’s say your shoulder hurts every time you press overhead.
Naturally, we want to assess the shoulder. But we may also need to look at what’s happening around it.
How is your neck moving?
What is your rib cage doing?
How well is your shoulder blade moving and controlling load?
Do certain positions change your symptoms?
Could a nerve be contributing to what you’re feeling?
The same concept can apply to low back pain, hip pain, sciatica, knee pain, and other persistent symptoms.
Instead of only asking:
“What structure hurts?”
We also want to know:
“What happens when your body actually moves?”
That gives us information an MRI alone cannot provide.
Could Your Nervous System Be Contributing to Your Pain?
Sometimes persistent symptoms involve more than muscles and joints.
Your nervous system plays an important role in how your body experiences and responds to pain. In some cases, irritated or sensitive nerves can contribute to symptoms such as aching, burning, tingling, weakness, tightness, or pain.
And nerve-related symptoms aren't always explained by imaging alone.
This is one reason someone may have already tried stretching, strengthening, massage, chiropractic care, physical therapy, or other approaches and still feel like something is being missed.
If treatment continues to focus primarily on where the symptoms are being felt, there may be other contributing factors that haven't been fully assessed.
What Should You Do If Your MRI Is Normal but You’re Still in Pain?
First, a normal MRI can be reassuring. Imaging can help rule out certain conditions or identify structural concerns that may require medical attention.
But if your MRI shows nothing concerning and you're still experiencing pain, that doesn't necessarily mean the conversation is over.
It may mean it's time to gather different information.
A more complete assessment may look at questions like:
How do you move when symptoms appear?
Are there certain positions that make your symptoms better or worse?
How does your body respond to different types of load?
Are other areas of your body influencing the painful area?
Could your nervous system be contributing?
What activities can you currently tolerate—and where do things start to break down?
Those answers can help create a clearer picture of what may be contributing to your symptoms and what your next steps should look like.
Your MRI Is One Piece of the Puzzle
When you’ve been dealing with pain for weeks, months, or even years, hearing that your MRI looks normal can be frustrating.
But a normal scan doesn't mean your symptoms aren't real.
And it doesn't mean you've run out of options.
Sometimes the next step is to stop looking only at what your body looks like and start looking more closely at how your body functions and responds to movement.
