Why Are We Testing Your Eyes When Your Hip Hurts?

You came in because your hip hurts.
Or maybe your squat feels restricted. Your low back keeps bothering you. You can't rotate as well to one side.
So why are we suddenly asking you to keep your head still and follow a target with your eyes?
It can seem completely unrelated.
But your eyes don't just help you see. The information coming from your visual system is also part of how your brain understands where you are in space and organizes movement.
That's why, at Fearless Movement Rehab, you may see us test someone's movement, have them perform what looks like a simple eye drill, and then immediately test that same movement again.
Sometimes nothing changes.
Sometimes the difference is noticeable within seconds.
And when something does change, that gives us information we can use.
What Do Your Eyes Have to Do With Movement?
Movement isn't controlled by your muscles and joints alone.
Your brain is constantly receiving sensory information to understand where you are, what's happening around you, and how your body needs to respond.
Three important sources of that information are your:
Visual system — information coming through your eyes about your environment and your position within it.
Vestibular system — information from your inner ear that helps your brain understand head movement, orientation, and balance.
Proprioceptive system — information from your muscles, joints, and other tissues that helps your brain understand where different parts of your body are positioned.
Your brain integrates this information and uses it to help coordinate your movement.
You don't consciously think about any of this when you walk across a room, balance on one leg, turn your head, squat, or rotate through a golf swing.
But your nervous system is processing information constantly.
That gives us another area to investigate when someone's movement isn't working the way they want it to.
So Why Would We Use an Eye Drill for Hip Pain?
This is where things can look a little weird from the outside.
Let's say we assess your ability to shift your hips to one side.
Maybe the movement feels restricted or looks noticeably different compared with the other direction.
We could immediately assume:
“The hip is tight.”
And sometimes addressing the hip directly may be appropriate.
But we can also ask another question:
What happens if we change the sensory information your brain is receiving and then test the movement again?
We might have you perform a specific eye movement for a short period of time.
Then we immediately retest your hip shift. If your movement changes, that's useful information. The eye drill didn't stretch your hip. It didn't suddenly strengthen a weak muscle.
And it doesn't mean we've “fixed” your hip in 30 seconds. What we've done is changed an input and observed whether the movement output changed with it.
That's the part we're interested in.
Test → Drill → Retest
This is one of the most important things to understand about the drills you see us use at Fearless Movement Rehab.
We're not choosing a random eye exercise because we saw it on Instagram and telling everyone with hip pain to do it.
We want to know how your body responds.
The process may look something like this:
1. TEST
First, we establish a baseline.
Maybe we're looking at your hip shift, squat, neck rotation, balance, shoulder movement, or another movement related to what you're experiencing.
How does it look?
How does it feel?
Is one direction different from the other?
2. DRILL
Next, we introduce a specific drill based on what we're assessing.
That may involve eye movement, head movement, balance, breathing, joint movement, strength, or another input.
3. RETEST
Then we repeat the original movement.
Did it improve?
Did it stay the same?
Did it get worse?
All three responses give us information.
Instead of assuming an exercise is right for you, we're looking for evidence that your body actually responds well to it.
Why Can a Movement Sometimes Change So Quickly?
This is where people understandably get skeptical.
If your hip movement changes immediately after moving your eyes, how could anything meaningful have happened that quickly?
The important thing is understanding what didn't happen.
You didn't suddenly build muscle.
You didn't dramatically change the structure of your hip.
You didn't lengthen a tissue by several inches.
Instead, we changed some of the sensory information being provided to your nervous system and then observed whether your movement changed.
The nervous system can alter movement quickly because it is constantly processing information and adjusting how your body responds.
That immediate change doesn't tell us everything about why you've been experiencing pain or restriction.
But it can give us another clue.
An Immediate Improvement Is Information — Not the Finish Line
This distinction matters.
You've probably seen videos online where someone does one drill, immediately moves better, and the implication is:
Do this exercise and fix your problem.
That's not what we're saying.
If an eye drill improves your hip shift during a session, that's interesting.
Now we want to understand whether that response is repeatable, how it relates to your symptoms, and how it fits into everything else we found during your assessment.
An eye drill may become one part of your plan.
Or it may simply help point us toward something we need to investigate further.
The goal isn't to collect cool before-and-after videos. The goal is to use your response to make better decisions about your care.
Does Everyone With Hip Pain Need Eye Drills?
No.
And that's exactly why we test and retest.
Two people can walk into Fearless Movement Rehab with what sounds like the exact same problem and need very different things.
One person's hip restriction may respond to a visual drill.
Someone else may need to work directly on hip strength or mobility.
Another person's symptoms may change when we address their ankle, spine, breathing, balance, or something else entirely.
There's no single eye drill—or any other exercise—that everyone with hip pain needs.
The right drill is the one that makes sense for the person in front of us and moves them toward their goal.
Your Body Is More Than the Spot That Hurts
When your hip hurts, we absolutely care about your hip.
But we don't want to stop our assessment there if something else may be contributing to how you're moving or what you're experiencing.
Your muscles, joints, sensory systems, and nervous system aren't operating independently. They're constantly exchanging information to help you move through the world.
Sometimes looking at those relationships helps us uncover options that haven't been explored before.
So if you see us asking someone with hip pain to move their eyes, we're not ignoring their hip.
We're gathering more information about the system controlling it.
